Receive application forms from the Office of the Registrar, PUMHSW, to submit a demand draft for Rs. 2500/- in favor of the Vice-Chancellor. Application forms can also be downloaded from the PUMHSW official website www.pumhs.edu.pk.
The application should include a CV, three passport-size photographs, two certified documents of educational experience, CNIC, and residence/PRC/form.
